Parents and Guardians, please be informed that the Ministry of Education has provided consent forms for you to use to grant permission for your child/ward to receive the COVID -19 vaccine.
The forms, which can be filled on site, can be uplifted from the security office at the school between the hours of 9 am and 3 pm. You are asked to deposit the completed forms into the box which will be stationed at the aforementioned security office.
Those who are desirous of making inquiries about the vaccine are advised to contact the Ministry of Education for further information.
