LOWER SIXTH FORM APPLICATION

Lower 6 application forms can be uplifted from the Guard Hut, the General Office and the offices of the Deputy Head Teacher or Head Teacher.

Submission of application forms should be made to the offices of the Deputy Head Teacher or Head Teacher. Accompanying your application must be:

1. Two (2) recent passport size photographs.
2. The applicant’s birth certificate ( or Deed Poll if relevant).
3. A testimonial/ recommendation from the applicant’s Headteacher.
4. The applicant’s results slip when available.

Photocopies of the applicants birth certificate and results slip should also be submitted.
